Radius #3 Anchor Lock form. 2x6's on the flat vertical with the #3 Anchor Lock mounted on the 2x6. The 2x3 telespar rail holds shape, but is not part of the loading bearing member.

Forms assembled

On this particular job, we designed it with 3 ties in the concrete, and one "dry" over the top of the concrete.

Finished product. Note the true radius for exposed concrete.
